Guardianship is a court proceeding in which a judge gives someone who is not the parent, custody of a child, or the power to manage the child's property (called "estate"), or both.

Generally, both parties are required to complete the documents listed below and mail a copy to the other party. Full disclosure of all assets, debts, income and expenses is required at two stages in your case; the beginning (Preliminary) and end (Final).
Generally the same forms are used for filing a dissolution (divorce), legal separation, and nullity (anulment) of a marriage or domestic partnership. (What's the difference?)
